Integrated care CoP webinar: Sharing successes – chronic disease prevention and management

Early diagnosis and proactive management in primary and community care improve key system outcomes and drive patient impact for high-risk patients. In FY24/25, 12 OHTs had implemented integrated clinical pathways for HF and COPD and tested early approaches to CDPM models. By learning from their successes and challenges to date, we aim to reduce the burden of starting from scratch, accelerate the spread of best practices, and promote a common vision for Integrated Care. This session is designed for all OHTs to support the planning and implementation of local CDPM models aligned with the FY25/26 CDPM deliverables through peer learning. We’ll be joined by:
