This session will focus on a key area of heart failure care: palliative care integration. We are pleased to share insights from Dr. Leah Steinberg, Family Physician and Palliative Care Lead at Mount Sinai Hospital, who brings valuable experience in embedding palliative approaches into heart failure care to support patients with complex needs. This work aligns with Quality Statement 10 from the Ontario Heart Failure Standards: People with heart failure and their families have their palliative care needs identified early and are offered support to address their needs. Please share with your ICP clinical leads and front-line providers within your OHT, as their perspectives are essential to this work.
