RISE peer sharing and learning session on the process of primary-care attachment

Building on the June session ( (PC-AA)), this session aims to help OHTs to:
- Understand how attachment processes fit into the bigger picture of OHT/PCN efforts to improve PC-AA and strengthen primary care.
- Explore how to develop and optimize your own processes for attachment to foster on-going relationships between a small team of healthcare providers and a patient.
- Provide feedback on the existing and proposed RISE’s supports for OHTs/PCNs to accelerate PC-AA, including next steps in this new cycle of peer sharing and learning sessions
- Continue to learn from peer OHTs/PCNs on how they’re approaching this work.
Audience: those in OHTs/PCNs leading PC-AA work and those supporting OHT/PCNs in this work.
Please note, due to registration limits priority will be given to those in OHTs/PCNs leading PC-AA work.
