RISE continues to offer opportunities for all approved OHTs/PCNs to collaborate virtually through peer sharing and learning sessions focused on ongoing and emergent OHT/PCN priorities, including those established through the work of Ontario’s Primary Care Action Team. On 16 October, RISE will host a session (12:00 – 1:30pm) focused on primary-care access and attachment (PC-AA). This session invites OHTs/PCNs, regardless of their stage in PC-AA, to exchange advice and support. Specifically, it aims to help OHTs/PCNs:

  • learn approaches to understanding your attributed population’s needs, including the needs of those unattached to primary care
  • explore strategies to assess and understand the supply of and demand for primary-care providers and how this affects PC-AA approaches
  • exchange learnings with peer OHTs/PCNs about how they’re understanding (measuring) demand, supply and need and using this understanding to inform the mix of PC-AA approaches to leverage as they design their interprofessional primary care team models.

If you are an OHT/PCN and would like to share your PC-AA learnings at an upcoming session or if you have questions/feedback on these sessions, please contact leslie.mcgeoch@outlook.com or your RISE coach.
