Peer sharing and learning session on an introduction to optimizing team-based primary care

RISE continues to offer opportunities for all approved OHTs/PCNs to collaborate virtually through peer sharing and learning sessions focused on ongoing and emergent OHT/PCN priorities, including those established through the work of Ontario’s Primary Care Action Team.
On 20 November, RISE will host a session (12:00 – 1:30pm) focused on an introduction to optimizing team-based care. This session invites OHTs/PCNs, regardless of their stage in primary care access and attachment, to exchange advice and support. Specifically, this session will provide an overview of drivers for developing a high performing primary-care system followed by a specific focus on one of these drivers (optimizing team-based care). This session is meant to aid OHTs/PCNs in addressing their TPA requirements.
