Pathways to preservation: Advancing Indigenous-led lower limb health

This webinar supports Ontario Health Teams (OHTs) in enhancing culturally safe health services for First Nations, Inuit, Métis, and Urban Indigenous (FNIMuI) populations. Since their introduction in 2019, OHTs have aimed to connect the healthcare system and have launched integrated clinical pathways for chronic conditions and mental health. Hosted by the Indigenous Primary Health Care Council (IPHCC) with Ontario Health, the session features Indigenous organizations sharing insights on wholistic, Indigenous-led approaches. This webinar will showcase the work of three Indigenous Health Service Provider organizations and their achievements in the demonstration phase of the Lower Limb Preservation Strategy. Two of the IPHCC’s member sites, Maamwesying North Shore Health Services, and Mamaway Wiidokdaadwin Primary Care Team, and one of our health system partners, the Indigenous Diabetes Health Circle’s will share their stories around the Lower Limb Preservation Strategy and the work they have completed to date through an Indigenous-led and delivered approach and how we can continue to improve in a good way.
