Ontario Health Teams are working hard to improve patient care, experience, and overall health outcomes across the continuum of care. That’s why more are prioritizing caregiver inclusion and support: because when caregivers are equipped as essential partners in care, patient health outcomes improve, care transitions and coordination are more successful, and providers experience less pressure. On September 16 at 12 PM, join our webinar to learn how Great River OHT and its partners are strengthening caregivers’ role in care across hospitals, community organizations, and long-term care settings, by:
